Indiana Opts into BALANCE: Medicaid to Pay $85.16 of $245/Month for GLP-1 Obesity Coverage as the Coverage Map Splits Two Ways
Public-payer GLP-1 coverage expanded again — this time in Medicaid. Indiana Governor Mike Braun directed the Indiana Family and Social Services Administration (which administers Medicaid for 1.9 million Hoosiers) to opt into the CMS BALANCE model, the federal demonstration that negotiates manufacturer discounts in exchange for state coverage of GLP-1s for obesity.
The concrete economics, per the Indiana Capital Chronicle brief:
- "Indiana Medicaid will pay $85.16 of the $245 monthly cost per person, thanks to contributions from the federal government and manufacturer discounts."
- Manufacturers agreed to provide free lifestyle support alongside the medication.
- Eligibility criteria and timing are not yet public — "the agreement is still being finalized."
Eli Lilly's chair/CEO David Ricks endorsed the move in the release, tying it to both health and labor economics: "Here in Indiana, 71% of adults are living with obesity or overweight. This toll on our state's health is also a toll on our economy, costing an estimated $10.7 billion a year, or nearly 2% of GDP." He explicitly named "breakthrough medicines like Foundayo and Zepbound" becoming available to eligible Medicaid patients.
The split map. As ObesityIntel's coverage notes, at the start of 2026 only 13 state Medicaid programs covered GLP-1s for obesity — down from 16 the year before (California, New Hampshire, Pennsylvania and South Carolina all removed coverage on budget grounds). Indiana's opt-in shows the two-sided dynamic: standalone state coverage collapses at list price, while the federal BALANCE model makes it viable by cutting the net price to ~$85/month. The result is a coverage map splitting between states inside negotiated-discount models and states outside them — the Medicaid complement to the $50/month Medicare GLP-1 Bridge (see CMS Launches Medicare GLP-1 Bridge Program, Extending Access to December 2027 Amid BALANCE Model Delay and the ~700K-senior enrollment tracking in Indiana Opts into BALANCE: Medicaid to Pay $85.16 of $245/Month for GLP-1 Obesity Coverage as the Coverage Map Splits Two Ways).
Why it matters. Each BALANCE opt-in converts a zero-reimbursement population into a discounted-volume one for Lilly and Novo, and each expansion strengthens the political floor under GLP-1 pricing even as commercial employers pull back (Employers Roll Back GLP-1 Coverage for Weight Loss as Exploding Demand and New Oral Options Fuel Cost Concerns).
