Medicare GLP-1 Bridge Reaches 700K Seniors at $50 Copay; Lilly Cuts Foundayo Starter Doses to $149

U.S. GLP-1 reimbursement and cash pricing continued to reset this cycle, on top of the previously recorded TrumpRx tiers ($149–$299/month) and Novo's $675 list-price cut effective Jan 1, 2027 (standing watch). The Medicare GLP-1 Bridge program — live July 1, 2026, offering eligible Part D seniors GLP-1 obesity drugs at a $50 monthly copay — has reached ~700,000 seniors, and Lilly CEO David Ricks says Lilly is capturing "7 out of 10" Bridge patients, many on Zepbound, while Foundayo draws Medicare patients "looking for convenience but not necessarily a huge weight-loss effect." That makes the Bridge the single biggest public-reimbursement opening for obesity drugs to date — and a Lilly-skewed one so far.

On the cash side, Lilly cut Foundayo's starter-dose self-pay pricing effective September 1: $149/month for the 0.8 mg and 2.5 mg doses and $199 for 5.5 mg — framed by Leerink as "an elasticity experiment and as a push to the first maintenance dose." Jefferies estimates ~80% of Foundayo scripts remain cash-pay with most patients still on starting doses, so these price points are effectively the market's marginal clearing price for oral GLP-1 trial starts. The structural backdrop: half of Wegovy and Zepbound sales are now direct exchanges with cash-paying patients, and "Wegovy's cash price has dropped 75 percent since 2021 as drugmakers compete directly for patients1" (The Daily Economy, Oct 2) — from the $1,349 launch list price to $149–$299 street tiers.
