FDA Grants Full Approval to Lilly's Inluriyo + Verzenio All-Oral Combo in ESR1-Mutated Metastatic Breast Cancer

On September 18, 2026, Eli Lilly announced FDA full approval of Inluriyo (imlunestrant), an oral estrogen receptor antagonist / oral SERD, in combination with Verzenio (abemaciclib), a CDK4/6 inhibitor, for adults with ER-positive, HER2-negative, ESR1-mutated locally advanced or metastatic breast cancer with progression after at least one line of endocrine therapy. It is Inluriyo's second FDA approval in under a year (monotherapy was approved September 2025). (Lilly release)

The approval converts the EMBER-3 combination data into a full (traditional) approval — not contingent on confirmatory trials — based on the Phase 3 EMBER-3 ESR1-mutant cohort (n=159): switching to the all-oral combo at clinical progression doubled progression-free survival, 11.1 vs 5.5 months (HR 0.53, 95% CI 0.35–0.80) versus Inluriyo alone, after an aromatase inhibitor with or without a CDK4/6 inhibitor.

Lilly oncology chief Jacob Van Naarden framed the commercial stakes: "In less than a year since its approval, Inluriyo is the leading treatment option for people with ER+, HER2–, ESR1m metastatic breast cancer, now reaching over half of all patients starting an oral SERD… Today's full approval extends what Inluriyo in combination with Verzenio can do for patients, with a regimen that has confirmed benefit… and doesn't introduce burdensome monitoring requirements." (Lilly release)

Why it matters: Fierce Pharma reads the move as Lilly "escalat[ing] the oral SERD battle" — locking up the ESR1-mutant niche with a confirmed-benefit, all-oral regimen as the crowded oral SERD field (AstraZeneca, Roche and others) matures. It also lands two weeks after AstraZeneca's camizestrant (Etcamah) won FDA approval on September 4 in the same broad ESR1-mutation setting — part of a dense September approval wave that includes FDA Approves Fayuvi — First-Ever Treatment for Sanfilippo Syndrome Type A, and Ultragenyx's Second Gene Therapy Approval in a Month and FDA Approves AQNEURSA for Ataxia-Telangiectasia — the First Drug Ever Approved for A-T Anywhere in the World.
