FDA Approves Tukysa (Tucatinib) Triple Therapy as First-Line Maintenance for HER2-Positive Metastatic Breast Cancer
October 7, 2026. The FDA approved tucatinib (Tukysa, Seagen/Pfizer) in combination with trastuzumab and pertuzumab "for the maintenance treatment of adults with unresectable locally advanced or metastatic HER2-positive breast cancer following induction treatment1" — moving the oral HER2-targeted tyrosine kinase inhibitor into the frontline maintenance setting and giving patients a chemotherapy-free maintenance option after induction.
The approval rests on HER2CLIMB-05 (NCT05132582), a randomized, double-blind, placebo-controlled trial in 654 patients who had no evidence of progression after 4–8 cycles of trastuzumab/pertuzumab/taxane induction (brain metastases allowed). Per the FDA: "Median PFS was 24.9 months (95% CI: 21.3, not reached) in the tucatinib arm and 16.3 months (95% CI: 12.6, 18.7) in the placebo arm (Hazard ratio 0.64 [95% CI: 0.51, 0.80]; p-value <0.0001)." Overall survival data were not mature at the PFS analysis. Dosing is 300 mg orally twice daily with trastuzumab + pertuzumab (IV or the subcutaneous fixed-dose combination with hyaluronidase) until progression or unacceptable toxicity; the label carries a boxed warning for hepatotoxicity plus warnings for diarrhea, embryo-fetal toxicity, and increased serum creatinine.
Pfizer chief U.S. commercial officer Aamir Malik framed the commercial significance: "Today's FDA approval marks the next chapter for [tucatinib], bringing it into the front-line maintenance setting and offering patients a chemotherapy-free option that can help delay disease progression after initial treatment." The approval extends Tukysa's franchise beyond its original later-line HER2CLIMB-02/HER2CLIMB settings, and follows this topic's recent run of breast-cancer regulatory actions (FDA Grants Full Approval to Lilly's Inluriyo + Verzenio All-Oral Combo in ESR1-Mutated Metastatic Breast Cancer, camizestrant watch context).
