U.S. Measles Outbreak and Elimination Status Crisis
The United States is facing an unprecedented resurgence of measles, with cases in 2026 reaching a 35-year high. The worsening crisis has placed the nation's official "measles elimination" status—which it has held since 2000—under formal and imminent threat of being revoked. An expert committee convened by the Centers for Disease Control and Prevention (CDC) is actively reviewing the epidemiological data in August 2026, ahead of a final determination by the Pan American Health Organization (PAHO) scheduled for November 2026.
A 35-Year High in Cases
As of mid-August 2026, the CDC has confirmed over 2,500 measles cases across 47 jurisdictions, officially surpassing the total for all of 2025 (2,289 cases) and marking the highest annual case count in the United States since 1991. Outbreaks are expanding rapidly in multiple states, including Wisconsin (which saw cases double to 36 in a single week), Iowa, and Oregon.
State epidemiologists and public health officials are sounding the alarm. Oregon State Epidemiologist Paul Sidelinger remarked:
"It is more than likely that come this fall, we will officially lose our measles elimination status as a country."
The Criteria for Loss of Elimination
For a country to maintain its "eliminated" status, it must demonstrate that there has been no continuous, sustained domestic transmission of the measles virus for 12 months or more. The ongoing review is dissecting several long-running outbreaks to determine if the chain of transmission has remained unbroken.
The primary area of concern is an active outbreak in Utah. Noel Brewster, PhD, who chairs the CDC review committee, stated in a Stat First Opinion piece:
"We will review measles rates for the nation. These data are public and show a runaway train... One of the most worrisome is the ongoing outbreak in Utah, which apparently has gone on for more than 12 months, the main criterion for loss of elimination."
Brewster added that losing this status would represent a historic and highly consequential public health reversal:
"Elimination status marks whether a country has stopped sustained domestic transmission. Losing that status would signal a consequential reversal, evidence that the systems and protections that once kept measles at bay no longer do."
Next Steps in the Review Process
The CDC committee is currently evaluating whole-genome sequencing data and conducting shoe-leather epidemiological tracing to map out the duration of the transmission chains. The final decision on whether the United States (as well as Mexico and Canada) will officially lose elimination status will be announced by PAHO, the regional branch of the World Health Organization, in November 2026.
