FDA Grants Accelerated Approval to Sanofi’s Tzield for Newly Diagnosed Stage 3 Type 1 Diabetes
On June 12, 2026, the U.S. Food and Drug Administration (FDA) granted expanded accelerated approval to Sanofi’s Tzield (teplizumab-mzwv) as the first-ever disease-modifying therapy designed to delay the decline of endogenous insulin production in pediatric patients aged 8 to 17 years who have been recently diagnosed with Stage 3 Type 1 Diabetes (T1D) (within the last 8 weeks).
This landmark approval represents a paradigm shift in type 1 diabetes care, offering newly diagnosed children and adolescents a therapeutic option besides insulin that targets the underlying autoimmune disease process. Tzield—an anti-CD3 monoclonal antibody—works by targeting the T cells responsible for destroying insulin-producing pancreatic beta cells, thereby slowing down the autoimmune attack.
Clinical Evidence and Surrogate Endpoints
The FDA's accelerated approval is supported by robust data from the Phase 3 PROTECT clinical trial. The trial demonstrated that Tzield safely and effectively preserves beta-cell function in newly diagnosed patients.
Crucially, this approval marks the first time a disease-modifying therapy for T1D has been cleared based on C-peptide, a validated surrogate biomarker of beta-cell function. In the PROTECT trial, patients treated with Tzield showed a statistically significant preservation of C-peptide levels compared to those on placebo, indicating that their bodies continued to produce significantly more of their own insulin. Preserving even a fraction of endogenous beta-cell function is highly clinically meaningful, as it translates to:
- Improved HbA1c levels
- Increased time-in-range (TIR)
- Reduced risk of severe hypoglycemic events
- Lower daily insulin requirements
Safety and Labeling
The approved labeling for Tzield includes a Boxed Warning regarding the risk of serious and life-threatening viral reactivation, specifically highlighting Epstein-Barr virus (EBV) and cytomegalovirus (CMV). The most common adverse events reported include vomiting, rash, headache, and elevated liver transaminases. Tzield is also associated with transient leukopenia, lymphopenia, and neutropenia, which can increase the risk of infections.
Ongoing Confirmatory Study
To verify the clinical benefit and secure full, traditional approval for this indication, Sanofi is actively conducting the Phase 3 BETA-PRESERVE (NCT07088068) confirmatory study. This clinical trial is evaluating the safety and efficacy of Tzield in a broader population of children and young adults (up to 25 years old) with newly diagnosed Stage 3 T1D. The trial's primary endpoints are HbA1c levels and the number of days patients can go without requiring mealtime insulin.
