FDA Expands Merck’s Capvaxive Approval to Include High-Risk Children and Teens
On June 17, 2026, the U.S. Food and Drug Administration (FDA) approved an expanded indication for Merck’s Capvaxive (pneumococcal 21-valent conjugate vaccine; PCV21). The vaccine is now indicated for the prevention of invasive pneumococcal disease (IPD) and pneumococcal pneumonia in children and adolescents aged 2 through 17 years who are at increased risk for pneumococcal disease and have completed a primary pneumococcal vaccine series.
This regulatory expansion represents a major public health advancement and a critical commercial win for Merck in the highly competitive pneumococcal vaccine market.
Commercial and Public Health Significance
Pneumococcal disease, caused by the bacterium Streptococcus pneumoniae, can lead to severe and life-threatening infections such as meningitis, bacteremia (bloodstream infection), and pneumonia. Children with chronic medical conditions—such as diabetes, chronic heart disease, chronic lung disease, or immunocompromising conditions—are at a substantially higher risk of contracting invasive pneumococcal infections.
Capvaxive is uniquely designed to target the specific serotypes that are responsible for the vast majority of invasive pneumococcal disease cases. Crucially, Capvaxive provides coverage for 11 serotypes that are not included in any other FDA-approved pneumococcal conjugate vaccine (such as Pfizer’s rival Prevnar 20). These 11 unique serotypes account for a significant portion of the remaining disease burden in high-risk pediatric populations who have already completed their standard childhood vaccine series (typically with Prevnar 15 or Prevnar 20).
By acting as a supplemental or booster dose, Capvaxive addresses a major gap in protection for chronically ill and immunocompromised children, offering broader coverage than standard childhood vaccination regimens can provide alone.
Clinical Data and Pediatric Studies
The FDA’s approval of the expanded indication was supported by pediatric clinical studies conducted under the Pediatric Research Equity Act (PREA). These studies evaluated the safety, tolerability, and immunogenicity of Capvaxive in children and adolescents aged 2 to 17 years who are at increased risk of pneumococcal disease. The clinical data demonstrated that Capvaxive elicited a robust immune response against all 21 vaccine serotypes, with a safety and tolerability profile comparable to its established profile in adults.
Capvaxive was originally approved by the FDA on June 17, 2024, for adults aged 18 years and older. This pediatric expansion occurs exactly two years after its initial market entry.
