BioNTech's mRNA Cancer Vaccine BNT113 Misses PFS Endpoint in HPV16+ Head and Neck Cancer — Overall Survival Now the Deciding Readout

October 13, 2026. BioNTech announced first topline results from the AHEAD-MERIT Phase 2/3 trial (NCT04534205): BNT113 (tenumomeran/raplemomeran) — an mRNA cancer immunotherapy encoding the HPV16 E6 and E7 oncoproteins — combined with pembrolizumab "did not demonstrate a statistically significant improvement in PFS versus pembrolizumab alone1" as first-line treatment for unresectable recurrent or metastatic HPV16-positive, PD-L1-expressing head and neck squamous cell carcinoma. The Independent Data Monitoring Committee recommended the trial continue to its second dual primary endpoint, overall survival — and the pre-specified statistical design means "a statistically significant improvement in overall survival would establish a positive trial outcome, even if the PFS dual primary endpoint was not met."

CMO Özlem Türeci: "While the PFS result was not statistically significant at this first analysis, the data support continued evaluation of BNT113 plus pembrolizumab in this trial and show that it is engaging its intended therapeutic mechanism… As immune-mediated anti-tumor effects build and deepen over time, the analysis of the second dual primary endpoint, overall survival, will be critical to fully assess the potential of this approach to deliver durable benefit for patients with HPV-driven head and neck cancer."

What it means: a setback for the most clinically advanced HPV-targeted therapeutic vaccine — there are still no HPV-targeted treatments approved despite ~a third of HNSCC being HPV-driven (~90% of oropharyngeal cases HPV16). The program survives on the OS readout; a miss there would be a major blow to fixed-antigen mRNA immunotherapy added to checkpoint blockade. BNT113 holds an FDA Fast Track designation (January 2026).

  1. An instance of PFS-driven accelerated approvals fail to guarantee durable overall survival benefits. — With PFS failing, only a durable overall-survival gain can validate the program, confirming OS as the binding test that PFS alone cannot satisfy. ↩︎
