Lilly's EloraTZP Amylin Combo Hits 23.3% Weight Loss in T2D Obesity — Topping Retatrutide in That Population — Phase 3 Planned Q4 2026

At EASD 2026 in Milan, Eli Lilly's amylin-based combination EloraTZP (eloralintide + tirzepatide) delivered the strongest obesity-plus-diabetes efficacy dataset the field has seen, per Lilly's September 30, 2026 release:

  • In the 48-week Phase 2b study (NCT06603571, 367 adults with obesity/overweight and type 2 diabetes), the highest dose combination (eloralintide 9 mg + tirzepatide 15 mg) produced -23.3% body weight (-54.1 lbs) and -2.9% A1C, versus -14.8% (-34.4 lbs) and -2.4% for tirzepatide 15 mg alone. Eloralintide 9 mg alone: -11.1%. Placebo: -3.0%.
  • "Participants taking EloraTZP lost up to an average of 54.1 lbs (23.3%) compared to 34.4 lbs (14.8%) with tirzepatide 15 mg" — all dose combinations met primary and secondary endpoints.
  • Fierce Biotech's EASD coverage noted the 23.3% figure is "comparable to tirzepatide in patients without T2D" and "notable given the efficacy attenuation typically seen with diabetes" (Citi analysts), and that it tops Lilly's own triple-G retatrutide in the T2D population.
  • The tolerability tradeoff is real: treatment discontinuation due to adverse events ran 10.8%–27.0% in the EloraTZP arms vs 2.9% for tirzepatide alone — GI events, mostly mild/moderate, concentrated during dose escalation. Lilly says it will use "an optimized escalation schedule" in Phase 3.
  • Kenneth Custer, president of Lilly Cardiometabolic Health: "EloraTZP represents our next frontier, pairing a selective amylin receptor agonist with tirzepatide, which could be an additive and more physiological approach to managing metabolic health."
  • Forward catalyst: "Lilly plans to initiate Phase 3 studies of an EloraTZP co-formulation product by the end of 2026."

Why it matters. This is the efficacy counterpunch in the amylin-combo race that defined EASD 2026 — and it lands just as Novo awaits a Q4 2026 FDA decision on CagriSema (see the CagriSema watch). EloraTZP's Phase 2b numbers in the harder-to-treat T2D population exceed what CagriSema showed in non-diabetic obesity (~22.7%), and Lilly is moving to Phase 3 immediately. The durability question — discontinuation rates up to 27% — is the bear counterweight, and the optimized-titration Phase 3 design is explicitly aimed at fixing it.
