Jazz and PharmaMar's Zepzelca Fails Confirmatory Phase 3 LAGOON Trial in Second-Line Small Cell Lung Cancer

The accelerated approval of Jazz Pharmaceuticals and PharmaMar's small cell lung cancer (SCLC) drug Zepzelca (lurbinectedin) is under intense regulatory scrutiny following the topline failure of its confirmatory Phase 3 LAGOON trial.12 On June 12, 2026, Jazz announced that the study failed to meet its primary endpoint of overall survival (OS) in patients with relapsed SCLC. Zepzelca monotherapy actually performed worse than standard chemotherapy, showing a 19% higher risk of death.3

This development represents the second late-stage failure for Zepzelca in the second-line setting, following the 2020 flop of the Phase 3 ATLANTIS trial. While the FDA previously allowed Zepzelca to remain on the market after the ATLANTIS failure due to dosing differences and high unmet medical need, the LAGOON trial tested Zepzelca at its approved 3.2 mg/m² dose, leaving little room for regulatory maneuvering. Zepzelca's second-line market share has already been declining due to competition from Amgen's recently approved T-cell engager Imdelltra (tarlatamab).

Key Findings & Data

  • Monotherapy Failure: Single-agent Zepzelca resulted in a median overall survival of 8.7 months compared to 10.7 months for the control arm (investigator's choice of topotecan or irinotecan), representing a hazard ratio indicating a 19% increased risk of death.
  • Combination Failure: The combination of Zepzelca and irinotecan showed a marginal 9.8% improvement in overall survival, which did not reach statistical significance.
  • CNS Subgroup Analysis: Even in patients without a history of brain metastases, single-agent Zepzelca underperformed compared to the control arm (9.6 months vs. 10.7 months).
  • First-Line Indication Unaffected: This failure does not impact Zepzelca's full approval (granted in 2025) in combination with Tecentriq as a first-line maintenance treatment for extensive-stage SCLC.

  1. An instance of Accelerated pathways shift the proof of clinical benefit from pre-market trials to post-marketing requirements. — Zepzelca's accelerated second-line SCLC approval is threatened after its mandated Phase 3 post-marketing confirmatory trial failed to establish an overall survival benefit. ↩︎

  2. An instance of Drug approvals are no longer permanent guarantees of market tenure. — This failure represents a direct threat to Zepzelca's market tenure, demonstrating how a post-marketing confirmatory study failure can strip a drug of its previously secured regulatory approval. ↩︎

  3. An instance of PFS-driven accelerated approvals fail to guarantee durable overall survival benefits. — The confirmatory trial failed to establish overall survival benefit, illustrating that early clinical progression metrics do not always translate into durable survival. ↩︎
