Oral GLP-1 Race: Foundayo's Q3 Inflection Confirmed, Then Pullback — Wegovy Pill Plateaus Near 180K Scripts
Fierce Pharma's Oral GLP-1 Tracker (updated Oct. 9, 2026, using IQVIA data cited by Citi and Jefferies) shows the oral battle entering a new phase: after Foundayo's long-awaited breakout, both pills pulled back in the most recent week — but the structural story is Lilly closing the gap.
The October inflection and pullback:
- Oct. 2 update: Foundayo weekly scripts surged ~21% to 64,268 (off a +13% week before) — a record. Jefferies (Oct. 2 note): "We're seeing early signs of growth in Q3 for Foundayo." Drivers cited: a Martha Stewart direct-to-consumer ad first aired in late August and the Medicare GLP-1 Bridge ($50 monthly copay for seniors, live since July 1).
- Oct. 9 update: Foundayo fell 9.9% to 57,920 in week 26; the Wegovy pill slipped 1.4% to ~179.5K. Scripts for all major GLP-1s declined that week except Zepbound KwikPen. Zepbound overall dipped below 800K (−0.8%); Novo's total Wegovy franchise (pill + injectable) fell 2.7% to 505.5K.
- The Wegovy pill has plateaued: weekly scripts have hovered between 181K–184K for five weeks (except a Labor Day lull). Foundayo needed 25 weeks to reach what the Wegovy pill achieved in 7 — but the gap is closing: Jefferies pegs Lilly at 26% of the oral obesity market, up from just under 23% two weeks prior.
- Revenue modeling: Jefferies now models $208M for Foundayo's Q3, in line with consensus — a sharp upgrade from the $71M they modeled for Q2 (actual Q2: $98M). Leerink notes Lilly cut entry-dose self-pay pricing to $149/$149/$199 effective Sept. 1, framed as an "elasticity experiment"; ~80% of Foundayo patients are still on starting doses and most scripts are cash pay.
- Class-level share: Lilly's tirzepatide holds ~57% of the total GLP-1 market (vs ~49.5% a year ago); Novo's semaglutide is flat at ~37%.
- Medicare Bridge concentration: Lilly CEO David Ricks (CNBC, late Sept.): Foundayo captures "about 1 out of 3 new starts for oral GLP-1 obesity drugs… that's growing week over week," and Lilly is capturing 7 of 10 Medicare Bridge patients.
Investor takeaways: (1) The oral segment is expanding the whole class — Citi's four-week GLP-1 script growth ran +5.4% vs a −6.8% decline a year earlier, attributed to orals broadening access. (2) Foundayo's DTC + Medicare Bridge playbook is working, but the Wegovy pill's plateau suggests the first-mover's ceiling — watch whether Novo responds on price or promotion. (3) Both majors report Q3 within two weeks: Eli Lilly on Thursday, Oct. 29 (BMO); Novo Nordisk on Nov. 4 — the Foundayo ramp, Wegovy-pill trajectory, and Medicare Bridge run-rate are the numbers to check (tracked in Medicare GLP-1 Bridge Hits 700K Seniors; Lilly Captures 70% at $245/Month Net — "Very Market-Expansionary").
