FDA Approves Enspryng (Satralizumab) for Thyroid Eye Disease — First At-Home Subcutaneous Option and a Second Indication for the IL-6R Inhibitor

October 16, 2026. Roche announced that the FDA approved Enspryng (satralizumab) for the treatment of adults with thyroid eye disease (TED) — "providing patients with the first and only at-home subcutaneous treatment option1," self- or caregiver-administered every four weeks during maintenance after training. It is the second indication for the anti-IL-6R antibody (approved for neuromyelitis optica spectrum disorder in 2020), granted under priority review.

The efficacy case rests on the Phase 3 SatraGO program (258 patients, 19 countries). SatraGO-2 met its endpoints in active TED — proptosis (eye-bulging) response 52.9% vs 23.4% for placebo, and diplopia (double-vision) reduction 60.7% vs 25.8% — while SatraGO-1 missed its pre-specified primary endpoint at Week 24; the approval cites the "totality of data," with consistent results in the overall population including chronic TED. No new safety signals vs the known NMOSD profile.

Roche CMO Levi Garraway: "Today's FDA approval of Enspryng provides an important new option for people with thyroid eye disease, who have long faced limited treatment choices... As the first subcutaneous treatment for this condition, Enspryng offers a positive safety and efficacy profile and the potential for at-home administration."

TED affects roughly 155 per 100,000 people in the US, about half of Graves' hyperthyroidism patients experiencing at least mild disease; untreated it can disfigure and threaten vision. Existing options (Teprotumumab infusions, steroids, surgery) are infusion-based or surgical, so an at-home self-injected biologic materially changes the treatment logistics. Pipeline note: an Enspryng sBLA for MOGAD is under priority review with a decision expected January 2027.
